Covid-19 Safety Plan Interaction Form

Please take the time to go over our Covid-19 Safety Plan Terms and Conditions carefully before completing the form.

This form is to acknowledge that you have read and understood the terms and conditions set out by Uzuri Hair Studio, in regards to our Covid-19 Safety Plan, Protocols and client register.

We ask that all clients and guest please read and follow the preventative and proactive conditions set out in the plan to ensure everyone’s wellbeing is taken into account.

Please read the terms and conditions of service carefully as the commencement of service by Uzuri Hair Studio is confirmation that you accept and fully understand all of the Terms & Conditions outlined below:

1.0  COVID-19 Term and Conditions of Service

1.1 For the purpose of these Terms and Conditions of Service, we, us, our, artist/s and stylist/s refer to Uzuri Hair Studio and our contracted stylists who are providing either hair and/or makeup services. Client, you and you’re refers to the person named in the form above

1.2 Once service commences by Uzuri Hair Studio, this form constitutes a binding agreement between the client (you) and Uzuri Hair Studio and/or our contracted stylist.

1.3 Uzuri Hair Studio reserves the right to alter these Terms and Conditions of Service and will notify you, the client, of any changes in writing if needed.

2.0 CLIENT SERVICE CONDITIONS

2.1 The client MUST consent to a contactless body temperature check before service commences.

2.2 The client MUST sanitise hands with provided hand sanitiser before engaging in the artist’s working area.

2.3 The client MUST not bring drinks, phones or any other item/s within the artist/s 4m² working area unless agreed by the artist/s prior.

2.4 The client MUST NOT touch the artist/s, their tools, items or equipment under any circumstance unless directed by the artist/s.

2.5  The client MUST NOT touch or physically interact with any other person/s while they are being serviced within the artist’s 4m² working area.

2.6 The client MUST disclose to Uzuri Hair Studio artist/s of any travel taken outside the Northern Territory within the last 7 days.

2.7 The client MUST disclose to Uzuri Hair Studio artist/s if they’ve been in contact with anyone who has tested positive to COVID-19.

2.8 The client MUST disclose to Uzuri Hair Studio artist/s if they are being tested for COVID-19.

2.9 The client MUST disclose to Uzuri Hair Studio artist/s if they are currently in mandatory Isolation/Quarantine.

2.10 The client MUST disclose to Uzuri Hair Studio artist/s if presented with any flu-like symptoms, sore throat, cough, fever or shortness of breath within the last 48 hours.

2.11 All makeup service clients may be subject to a RAT test 15 minutes prior to a makeup application

2.12 Uzuri Hair Studio reserves the right to refuse service to any and all clients who refuse to adhere to the above mandatory conditions of service. A refund is not applicable under any circumstance if service is refused due to breach of the conditions of service or Uzuri Hair Studio feels at risk of infection.

2.13 In the event a client/s is tested to have a temperature over 37.5°C, Uzuri Hair Studio reserves the right to refuse service to that client. Uzuri Hair Studio will request the client recording a high temperature of over 37.5°C to be removed from the room/area away from Uzuri Hair Studio Artist/s to avoid any possible contamination and spread of infection.

3.0 ARTIST/S WORKING AREA/CONDITIONS

3.1 Clients, friends, family and/or guests MUST   not come within 1.5 meters of Uzuri Hair Studio Artist/s and the client they are servicing. A 4m² area MUST be maintained from Uzuri Hair Studio Artist/s and the clients they are servicing AT ALL TIMES. Uzuri Hair Studio reserves the right to terminate the service at any time if these social distancing parameters are breached and/or if the artist/s feels at risk of infection due to unacceptable social distancing behaviour.

3.2  There cannot be any more than 10 people (including Uzuri Hair Studio artist/s) in the area at one time with a social distanced area of at least 4m² per artist MUST be available. If the working area is only big enough for Uzuri Hair Studio artist/s and the serviced client/s, all other people must be removed from the room. If a suitable working area is not provided and or there are too many people within the room, Uzuri Hair Studio reserves the right to terminate the booking at any time if these government legislated parameters are breached and/or if the artist/s feels at risk of infection due to unacceptable room capacity breaches.

4.0  COVID-19 WAIVER/LIABILITY

When choosing to commence hair or makeup services by Uzuri Hair Studio, you are doing so at your own risk. By agreeing to the terms and conditions of service, you agree that Uzuri Hair Studio cannot be held accountable for any infection or contamination relating to infectious disease either directly or indirectly from being serviced by us.

This includes but is not limited to *Accidental injury by contamination of infectious disease to persons including but not limited to, injury resulting from the use of tools and equipment required for the service, injury to the client, any of their friends, family or guests whether that person is part of the booking or not, and any other person at the location that the booking is taking place at. *Any other illness or infection incurred by circumstances outside of Uzuri Hair Studio’s control.

It is the client’s responsibility to disclose and inform the stylist/s if you present any flu-like symptoms, sore throat, cough, fever or shortness of breath in the last 48 hours or of any potential illnesses which can affect the wellbeing of the artist/s and or contaminate the equipment.

Uzuri Hair Studio Artist/s will take all possible precautions and uphold a strict and stringent hygiene procedure as per the State Government required COVID-19 Safe Plan to avoid and limit contamination and spread of infection. While all care is taken, no responsibility is accepted.
